Education Verification

Education Verification confirms an applicant’s credentials by verifying the highest level Degree or Diploma as related to your job description. Original Documentation is required. Please check with your recruiter as to what education requirement you are required to send a copy of.

Employment History/ Reference Request

Please make sure to fill out your DocuSign reference sheet with correct employment history dates, Supervisor contact information, and office numbers where we can reach them. Only PROFESSIONAL supervisor references are accepted

Hepatitis B Vaccination or Declination

Most clients require a copy of all three Hep B vaccines or a Hep B declination. Occasionally clients may require a Hep B titer. Please check with your recruiter for the specifics.

Physical/ Health Statement

Some clients require a recent physical with your physician. Please check with your recruiter for what is required for your specific job.

Positive PPD - Chest X-Ray

If positive PPD, you must submit a chest x-ray, generally within the past 1-5 years. Please check your job requirements.

PPD / Tuberculosis screening

Most clients require this and may ask for one of the following : IGRA (T-spot, Quantiferon Gold), PPD Skin Test, or 2 Step PPD within the past 12 months. Please check with your recruiter for your specific timeframe required.
